Important coverage information
Information about cyber liability, device protection, risk assessments, security services, incident response, or related products is general and does not create or guarantee coverage. Cyber insurance policies differ materially. Coverage is subject to application information, eligibility, underwriting, carrier approval, limits, sublimits, deductibles or retentions, waiting periods, conditions, exclusions, endorsements, territorial restrictions, sanctions, and other policy terms.
Policy wording controls
Only the policy as issued, including declarations, schedules, forms, endorsements, exclusions, and conditions, determines coverage. Marketing examples and summaries are illustrative and may describe coverage that is unavailable, optional, limited, or offered by a different carrier or form. No website statement expands or changes an issued policy.
Azyck's role
Azyck Cyber Insurance Inc. is contracted with a DOXA-affiliated entity and acts under delegated authority. Where expressly permitted by the applicable agreement, carrier appointment, underwriting guidelines, and law, that authority may include soliciting, quoting, underwriting within defined parameters, and binding coverage on behalf of the issuing insurer. Any authority is limited to authorized products, risks, jurisdictions, limits, and conditions and may be suspended or withdrawn. The issuing insurer is identified in the quote, binder, declarations, and policy and is responsible for the policy's obligations.
DOXA is a Goldman Sachs portfolio company and is backed by Goldman Sachs. Neither that investment relationship nor Azyck's contract with a DOXA-affiliated entity constitutes licensure by Goldman Sachs. The applicable state insurance regulatory authority issues any insurance license held by Azyck, and any producer appointment or binding authority arises from the applicable carrier and contractual authorizations. Goldman Sachs does not issue, underwrite, administer, or guarantee coverage solely because of its investment relationship with DOXA.
No binding or claim notice through general channels
Submitting a form, scheduling a meeting, receiving an assessment, proposal, quote indication, invoice, or marketing communication does not bind coverage. Coverage is effective only after authorized written confirmation. Do not report a suspected incident, claim, or circumstance through a public contact form, social media, or an appointment request. Follow the reporting instructions and deadlines in the applicable policy. Delay may affect available rights or coverage.
Applications and material information
Applicants are responsible for complete and accurate information, including security controls, prior incidents, loss history, known circumstances, systems, data, revenue, and operations when requested. Inaccurate, incomplete, or outdated material information may affect pricing, eligibility, rescission, cancellation, renewal, or claim determinations, subject to applicable law. Promptly report material changes through an authorized channel.
Cybersecurity and risk services are not a guarantee
Assessments, questionnaires, scans, recommendations, scores, monitoring, training, tools, or third-party services do not guarantee prevention, detection, containment, recovery, legal compliance, insurability, or claim payment. No control eliminates cyber risk. Azyck's services do not replace qualified legal counsel, a managed security provider, incident-response professionals, or the insured's own security program.
Incident-response and vendor services
Breach counsel, forensic, notification, credit monitoring, public relations, restoration, extortion response, and other vendors may be subject to carrier consent, panel requirements, rates, sublimits, and policy conditions. Incurring costs or retaining vendors before required approval may affect reimbursement. Follow the policy and authorize the incident-response process.
Availability and regulatory variation
Products, limits, pricing, services, and forms vary by state and applicant and may change. Some services may be provided by independent third parties under separate terms. Insurance is not a substitute for compliance with privacy, cybersecurity, notification, contractual, or industry obligations.
